What is the Sarnia-Lambton Ontario Health Team?
Ontario Health Teams (OHTs) provide a method of organizing and delivering patient care. Health Care Professionals from different organizations operate as one coordinated team to ensure that Ontarians receive more integrated care. There are 58 OHTs across the province, including one in Sarnia-Lambton.
The Sarnia-Lambton Ontario Health Team is a collaboration of 35 partners, including the County of Lambton, with a shared goal of improving the healthcare journey for members of our community.
