Report Adverse events following Immunization

Health care providers (i.e. physicians, nurses and pharmacists) are required by law to report AEFIs.

In the event of an AEFI:

1. Review the following documents:
• Adverse Event Following Immunization Fact Sheet
• AFI Provincial Case Definitions

2. Complete and return the Adverse Event Following Immunization (AEFI) Reporting Form to Lambton Public Health.
