Becoming a COVID-19 Vaccine Administration Site

Through honest and open discussions of benefits, risks and possible side effects, you help your patients and clients understand their most important health related choices.

Your clinic is interested in offering COVID-19 vaccine to your patients – next steps

1. Email our team to express interest in becoming a COVID-19 vaccine site

2. Complete COVaxOn training with OntarioMD

OntarioMD provides your clinic with access to:
• Training sessions- COVaxOn workflow training
• OHIP Billing guide
• Checklist- to prepare your clinic for vaccinations
• Job Aids -with step by step instructions
• Data Entry Forms- for offline recording of vaccine administration

4. Submit office information to Public Health

Once you have emailed Lambton Public Health to express interest in administering COVID-19 vaccines, you will be sent a User Load Template to register your staff for COVax.

5. Prepare your clinic

6. Order COVID-19 Vaccine

*Only submit an order if you have spoken to a Lambton Public Health staff member to confirm onboarding steps are complete. 

When ordering vaccine please remember the following: 
• Moderna vaccine must be used within 24 hours of puncture
• Pfizer vaccine must be used within 6 hours of puncture
• Refrigerated vaccine must be used within 30 days if un-punctured
• To the extent possible, plan the number of patients to vaccinate with the goal of limiting vaccine waste (moderna- 14 doses per vial, Pfizer- 6 doses per vial) 
• Only order the vaccine you are certain you can use
• Ideally the vaccine will be used as soon as possible (within a week)
