Becoming a COVID-19 Vaccine Administration Site

Health Care providers are a trusted source for public health advice. It is your one-on-one conversations about the benefits, risks, and possible side effects that build vaccine confidence in your patients. Through honest and open discussions, you help your patients and clients understand their most important health-related choices. Having the option to be vaccinated by their trusted provider in a familiar environment may be the only way for them to get vaccinated. 

If your clinic is interested in offering the COVID-19 Vaccine to your patients, please follow these steps.

1. Email our team to express interest in becoming a COVID-19 vaccine site

Email covaxsupport@county-lambton.on.ca to express your interest in providing COVID-19 Vaccine at your clinic location.

3. Submit office information to Lambton Public Health

*Only submit an order if you have spoken to a Lambton Public Health staff member to confirm onboarding steps are complete.

*Prior to submitting a vaccine order, fridge temperature logs must be submitted from the last 7 days by fax: 519-383-7092 or email: vaccineorders@county-lambton.on.ca

When ordering a vaccine please remember the following:
• To the extent possible, plan the number of patients to vaccinate with the goal of limiting vaccine waste
