School-Aged Immunization Clinics
Immunization Catch-Up Clinics for JK to Grade 12
Lambton Public Health offers school-aged catch-up clinics for those requiring vaccines who do not have a healthcare provider or are having difficulty obtaining these vaccines from their provider.

High School Catch-Up Clinics for Grade 9 to 12
Lambton Public Health will be offering catch-up clinics at high schools for students who received a First Notice letter in January 2024. All clinics will take place during school hours and are only for students attending the school hosting the clinic. Lambton Public Health provides free vaccinations to high school students that are overdue for mandatory vaccines (tetanus, diphtheria, pertussis, measles, mumps, rubella, and meningococcal) and recommended vaccines (human papillomavirus (HPV), hepatitis B and chickenpox). Informed consent for vaccine administration will be obtained from the student and assessed by the on-site Public Health Nurse. If you know a student is unable to provide informed consent, please do not book into a high school clinic; see your health care provider or book an appointment at a catch-up clinic with a parent/guardian as an alternative. Students are encouraged to discuss immunizations with their parents/guardians.

Grade 7 School Immunization Clinics
Lambton Public Health is back in schools providing vaccinations. Due to the COVID-19 pandemic, vaccinations will be available for both grade 7 and 8 students in the 2023/24 school year. It is important to read the parent/guardian instructions below and review the vaccine-specific fact sheet prior to completing and submitting the online consent form.

Routine Immunization Clinics

COVID-19 Vaccine Clinics
Lambton Public Health is currently offering Pediatric COVID-19 Immunization Clinics for individuals ages 6 months to 11 years old. If you are 12 years and older, please visit a participating pharmacy or your health care provider for COVID-19 and/or influenza vaccines.

School Suspension FAQ
Why did I get an Immunization Information Notice letter in the mail?
This letter means that according to Lambton Public Health (LPH) records, your child is missing a required vaccine that is part of Ontario’s Publicly Funded Immunization Schedule. This may be because:
a. Your child received the vaccine, but it has not been reported to LPH.
b. Your child has not received the vaccine.
c. Your child received the vaccine, but it was not at the right time.
d. A valid medical or conscious/religious exemption has not been submitted to LPH.
What immunization information is required by law?
According to Ontario’s Immunization of School Pupils Act (ISPA), students must be fully immunized against the following diseases according to Ontario’s Publicly Funded Immunization Schedule and the immunizations must be reported to Lambton Public Health:
• Diphtheria
• Tetanus
• Pertussis (whooping cough)
• Polio
• Measles
• Mumps
• Rubella
• Meningitis (meningococcal disease)
• Varicella (chicken pox) (for children born in 2010 and after)
My child has already had a required vaccine. What should I do?
Report these vaccines to Lambton Public Health. Check your child’s immunization record (yellow card) or call your health care provider for this information. Vaccines can be reported:
• Online using Immunization Connect Ontario (ICON)
• Mail/Drop off a copy: Immunization Records, 160 Exmouth St., Point Edward, ON N7T 7Z6
When reporting online, please upload a copy of your child’s record. If you are having trouble reporting online or have questions for a nurse, contact our Vaccine Call Center at 226-254-8222.
Do health care providers report immunizations to Lambton Public Health?
Health care providers are not required to report immunizations to Lambton Public Health. According to the Immunization of School Pupils Act (ISPA), it is the parent’s responsibility to report their child’s immunizations when attending daycare and/or school. While some health care providers may indicate that they report immunizations to public health, it is the parent/guardian’s responsibility to check that the records are complete. What should I do if my child requires one or more vaccines?
All required immunizations are free. You can receive missing vaccines from:
a. Your primary health care provider. Contact your provider to book an appointment for an immunization. Bring the letter you received in the mail. Once the vaccine is received, report it to Lambton Public Health.
b. Lambton Public Health will be hosting clinics at high schools and in the community. Book an appointment online.
My child cannot receive the required vaccine(s) for health reasons. What should I do?
You must have your health care provider complete a Medical Exemption Form and provide this record to Lambton Public Health. Exemption Instructions
I do not want my child to receive the vaccine(s) due to conscience/religious beliefs. What should I do?
You are required to complete an Immunization Education Session and a Statement of Conscience or Religious Belief Form. The form must be signed by a commissioner for taking affidavits in Ontario. Both the Vaccine Education Certificate and Form must be submitted to Lambton Public Health. Exemption Instructions
Are COVID-19 vaccines required for school and do they need to be reported to Lambton Public Health?
No, COVID-19 vaccines are not currently required to attend school and they do not need to be reported.
