Lambton Public Health works to support and assist health care professionals in their daily work with health information, community resources, and educational services that are integral to their role as they support individuals with health issues.
Community Paramedic Program
The Community Paramedic Program is provided by the County of Lambton’s Emergency Medical Services (EMS) Department in partnership with the Ministry of Long-Term Care, Ontario Health, and Bluewater Health.
The Program is designed to assist high users of EMS and the health system to live independently at home. The paramedics assigned to this program operate in an expanded role, outside traditional emergency response and transport and have received additional training.

Community Paramedic (CP) Programs and Services
The role and scope of the Community Paramedicine program is continually evolving. Community Paramedics provide comprehensive in-home health assessments, including checking vital signs and blood work, providing vaccinations, and helping patients understand how to manage their health conditions.
Service Hours: 7am-7pm, 7 Days a week
1. Community Paramedic (CP) Home Visiting Program
Referrals completed by: Primary Care Practitioners, Ontario Health at Home, and EMS
Services Include: Physical assessments, Fall risk assessments, Home safety scans, Continuous care planning, medication review with the client, and consultation with primary care providers for medication refills and referrals.
Diagnostics and Treatments Available: Blood draws, Urinalysis, influenza vaccines, and covid vaccines, iStat CHEM8+ chemistries/electrolytes, INR, 12-lead ECG.
Visits: Are scheduled and occur during the day in the patient’s community setting. The frequency of visits is based on the patient’s physical health condition and medical needs.
Cost: There is no cost to enroll in the program.

2. Community Paramedic (CP) Hospital Transition to Home Program
Eligibility: for clients returning home from the hospital, short term home visiting program to help acclimate patients back home after a recent admission to hospital.
Referrals completed by: Bluewater Health
3. Community Paramedic (CP) Long-Term Care Eligible Program
Eligibility: for clients on the waiting list for long-term care home placement.
Referrals completed by: Ontario Health at Home or primary care practitioner.
4. Community Paramedic (CP) Community Wellness Clinics
Current clinics:
• Florence, ON at St Matthews Hall – the last Wednesday morning of the month
• Camlachie Exercise Group at Camlachie Community Center – First Monday morning of the month
• Arkona Social Group at Orchard View Apartments – First Friday morning of the Month
• Thedford Exercise Group at Thedford Arena – Third Friday morning of the Month
• Kettle and Stoney Point Social Group at Elders Lodge – Third Monday morning of the Month
• 2 clinics in Sarnia located at County of Lambton buildings
Services include: Assessments, Vital signs, Medication Review, Health Promotion and Education, Health System Navigation, possible referrals to other community programs as needed.
Community Wellness Clinic Video
Cool Aid Program (Fridge-Magnet)
The program provides patients with a fold-up, magnetized form that contains medical information. Inside the form is a space for patients to record personal information, medical issues, prescribed medications, and allergies. The card is then placed on the outside of the individual’s fridge.
Lambton Paramedics are trained to look for this form when responding to calls for medical assistance.

Sarnia-Lambton Ontario Health Team
Ontario Health Teams (OHTs) provide a method of organizing and delivering patient care. Health Care professionals from different organizations operate as one coordinated team to ensure that Ontarians receive more integrated care. There are 58 OHTs across the province, including one in Sarnia-Lambton.
The Sarnia-Lambton Ontario Health Team is a collaboration of 35 partners, including Lambton Public Health, with a shared goal of improving the healthcare journey for members of our community.
