Reporting Forms and Resources

Diseases of Public Health Significance Reporting Requirements

Schools must report suspected or confirmed Diseases of Public Health Significance to Lambton Public Health within one business day of becoming aware of the illness.

Reportable Diseases of Public Health Significance List

Reporting is required if you suspect or confirm any of the specified diseases or events (as per Ontario Regulation 135/18 and amendments under the Health Protection and Promotion Act).

How to Report:

Fax: 519-383-7092 (During business hours only, 8:30 a.m. – 4:30 p.m. Monday to Friday)

Increased Absenteeism

Schools should report illness-related absenteeism when:

  • 20% or more of students and/or staff are absent due to illness, or
  • Illness-related absenteeism is higher than expected within a classroom, grade, or other group, even if overall absenteeism is below 20%

How to Report:

  1. Complete the School Absenteeism Reporting Form
  2. Include all requested information
  3. Fax the completed form to Lambton Public Health at 519-383-7092
  4. Once submitted, an Infectious Disease Prevention and Control team member will contact the school with any required follow-up or additional guidance

Classroom Resources

Glow Germ Kit

The Glow Germ Kit is an interactive teaching resource that demonstrates how germs spread and reinforces proper handwashing and surface cleaning techniques using fluorescent lotion and a handheld UV light.

Schools can:

  • Reserve a Glow Germ Kit from Lambton Public Health
  • Download classroom activity instructions
  • Use the kit to support hand hygiene education and infection prevention activities
